17-Hydroxyprogesterone (17-OHP) is a steroid hormone produced by the adrenal glands and, to a lesser extent, by the ovaries and testes. It is a precursor in the synthesis of cortisol and androgens, making it a crucial marker in evaluating adrenal gland function and diagnosing congenital adrenal hyperplasia (CAH). CAH is a group of genetic conditions that affect the adrenal glands’ ability to produce cortisol efficiently, leading to an overproduction of 17-OHP. Elevated levels of 17-OHP can suggest CAH, prompting further genetic testing and potentially life-long management strategies. This test is particularly important in newborn screening to detect CAH early in life, which can prevent severe illness or death. It is also used in adults to investigate causes of infertility, testicular tumors, and adrenal tumors.
